Pay Rate: $25-$27 per hour

The Coordinated Family Support (CFS) Specialist provides assistance with setting up and coordinating the services and support needed to allow adult clients to continue living in their family home. This includes developing skills in the home as well as coordinating the delivery of needed services including; helping with “generic services” and helping clients access their community. Support will be provided both in the client’s home and within the community.


EssentialDutiesandResponsibilities:

  • Demonstrates competence in performingthe necessary skills andperforms those skills listed on the service plan
  • Use of phone and email to assist client in researching and setting up services.
  • Communicates plans and progress with the client’s family
  • Mayassist insupervisionof client during community outings
  • Provides support necessary to successfully reside in the family home
  • Provides assistance and training to the consumer and their family with navigating services and support
  • Assists with scheduling of service delivery including medical and other appointments
  • Prepares and submits quarterly reports on progress of service delivery
  • Strictlyadherestoconfidentialityrequirements
  • Appropriatelyreportschangesinclientstatusorasdirectedbycareplan
  • Maintainsconfidentialityof allinformationpertainingtoclients,familiesandemployees
  • Performsotherdutiesasassigned/necessary


MinimumRequirements:

  • Statelicensureorcertificationasrequiredbystateand/orprogram
  • Associates level degree in a human services field of study OR 3 years of experience in the developmental disabilities service delivery system
  • CurrentBLSCard or CPR and First Aid training certification
  • CurrentPPDorChestX-Ray
  • Abilitytocoordinateandfacilitateservicesbetweenthehome,school,localhealthagency,and other community resources
  • Abilitytoeffectively elicit/provideinformation to andfrom appropriate individuals(including,butnotlimitedto, supervisors, co-workers,clients)viastrong communicationskills;proficiencyinthe Englishlanguagemayberequired
  • Computerproficiencyrequired
  • Mustbeat least18years ofage
