Maxim Healthcare Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for an adult patient in their home in Berwick, PA.
This rewarding opportunity allows you to build meaningful relationships with your patient and family while delivering high-quality, personalized care in a comfortable home setting. Responsibilities
Provide direct one-on-one nursing care for an adult patient in the home
Monitor and assess the patient's condition and report changes as appropriate
Administer prescribed medications, treatments, and therapies
Follow physician orders and individualized care plans
Assist with disease management and health maintenance
Maintain accurate and timely nursing documentation
Educate and support patients and family members as needed
Ensure patient safety and promote optimal health outcomes
Communicate effectively with the clinical team and caregivers
Qualifications
Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing
Current CPR/BLS certification
Previous homecare experience preferred but not required
Strong assessment and communication skills
Ability to work independently in a home setting
Reliable transportation
