We are seeking LVNs interested in caring for patients requiring:
G-Tube Care
Tracheostomy Care (Trach)
Ventilator Management
Pediatric Nursing
Medication Administration
Chronic Disease Management
Skilled Home Health Nursing
Experience with Trach, Ventilator, G-Tube, and Pediatric Patients is highly preferred but not required. Training opportunities are available for qualified candidates.
Responsibilities
As a Home Health LVN, you will:
Provide skilled nursing care in accordance with physician orders
Perform patient assessments and monitor changes in condition
Administer medications and treatments
Provide tracheostomy, ventilator, and enteral feeding care
Document patient care accurately and timely
Communicate with physicians, families, and clinical teams
Educate caregivers and family members
Support safe, compassionate, patient-centered care in the home
Ideal Candidates
Active California LVN License
Current CPR/BLS Certification
Strong communication and clinical assessment skills
Passion for patient advocacy and quality care
Home health, pediatric, trach, vent, or G-tube experience preferred
