Responsibilities
Deliver skilled nursing care in a home setting
Administer medications, treatments, and physician-ordered interventions
Monitor patient conditions and report changes to the care team
Implement individualized care plans
Complete accurate documentation and charting
Collaborate with physicians, caregivers, and interdisciplinary healthcare teams
Required:
Active Licensed Practical Nurse (LPN) license
Current BLS/CPR certification
Preferred:
1+ year of nursing experience
Experience with tracheostomy (trach), ventilator (vent), G-tube, enteral feeding, seizure management, or other skilled pediatric/homecare cases preferred but training available
