Your Role as an LPN
In this role, you will provide high-quality, patient-centered care in a home setting:
Deliver skilled nursing care in accordance with physician orders and care plans
Assess and monitor patient conditions, recognizing changes in physical or emotional status
Collect, analyze, and document patient data to support care planning and coordination
Assist in developing and implementing individualized care plans
Administer medications and treatments as prescribed
Educate patients and families to support safety, independence, and overall well-being
What We’re Looking For
Active Licensed Practical Nurse (LPN) license in Massachusetts
Current BLS/CPR certification
Current TB test or chest X-ray
Strong clinical skills and attention to detail
Compassionate, patient-focused approach to care
One year of nursing experience preferred (new graduates encouraged to apply)
