Transition Team Supplemental Care

Help patients transition from hospital to home care 

Our Transition Team is a supplemental care unit of skilled nurses whose primary function is to quickly and efficiently get patients home while primary nursing staff are actively recruited. This team is deployed after we receive a patient referral for home care services and the patient is admitted to Maxim.

Transition Team nurses provide care for all authorized hours in a home during a provisional period between 1-120 days. Once the full-time replacement nurses are trained and have a schedule, the Transition Team nurses are phased out of the home and onto their next assignment.
