Your Role
As a Homecare Nurse, you will provide individualized, patient-centered care while working closely with patients and their families:
Deliver skilled nursing care in accordance with physician orders and care plans
Assess, monitor, and document patient conditions, identifying physical and psychosocial changes
Administer medications, treatments, and interventions, evaluating effectiveness
Develop, implement, and adjust care plans based on patient needs (RN scope)
Collect and interpret clinical data to support high-quality outcomes
Communicate changes in patient status to physicians and care teams
Educate patients and caregivers to promote independence, safety, and wellness
Qualifications
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts
Current BLS/CPR certification
Current TB test or chest X-ray
Strong clinical skills, attention to detail, and a patient-first mindset
One year of nursing experience preferred (new graduates encouraged to apply)
